Making a request
All requests can be made using our request form. If you do not wish to print or use our form, we can still process your request. We will need a full name (first, middle and last), Date of birth (mm/dd/yyyy) and where they have resided in the last 7-10 years, 7 being the minimum for a completed report (Address must include city, state and zip code). If you cannot disclose the full address, we will require the city/state/zip code to process the request. For a motor vehicle request, we will require a driver's licence number. If you do not provide 7 years of residential history, we will still be able to process the report with an "incomplete" status and will provide you with all available results.
